Every player will need to fill out the 2 forms below before they are allowed to participate.

 

 

Application Form 

 

Designed to give high school teams the opportunity to compete against quality competition in one of the Midwest’s finest facilities. Teams can choose between a 1 or 2 day team camp. Teams are guaranteed 3 games each day. Each participant competing

in camp must fill out an application form.  Teams wishing to stay in the residence halls are responsible for reserving their rooms. The cost is $18 per person per night. Call (605) 688-5148 to reserve a room. Each team that stays in the dorms must provide a coach to stay in the hall during camp. A/C door rooms are available for $35 a night per person, please ask about availability at (605) 688-5148.

 

Each Team Camp Participant needs to fill out the application form completely. Each participant and their parent or guardian must read and sign a release form that will be sent once the application and payment are received. The application forms will be sent to:

If a camper will require any accommodations due to special needs, we ask that you notify us via email or in writing of those needs two weeks prior to the camp start date in order for accommodations.
